Team Leader

The Team Leader is responsible for the oversight and supervision of related staff including daily operations, coordination, vehicle maintenance, scheduling and overall management of assigned team in our Day Habilitation Program.

  • Works under the direct supervision of the Director to take a leadership position within a given group by presenting a positive role model for other staff to follow.
  • Develops, coordinates, plans, manages and supervises instructional services within assigned team.
  • Supervises the daily activities of assigned staff, performs site visits, implements programs that may include a combination or variety of training and monitoring at a community worksite, and/or plans and executes groups and individual activities utilizing recommended interventions and teaching procedures.
  • Observes and assists staff to implement teaching procedures and ensures that services are delivered in a manner that is responsive to the interests and desires of each person receiving support.
  • Verifies service delivery and ensures accurate daily billing, completion of monthly notes and Staff Action Plans, and ensures that the most appropriate, individualized and ethical programs are implemented and accurately documented.
  • Ensures delivery of Day Habilitation, Pre-vocational, Pathway to Employment, Supported Employment, and other OPWDD supports.
  • Depending upon assignment, the Team Leader may be required to work flexible hours, including nights and weekends, based on programmatic needs.

Minimum Education and Experience:

  • Bachelor’s Degree in Human Services or related field. Or a combination of related education and supervisory experience in the I/DD field totaling five years or more required.
  • Two years of pertinent experience required or Master’s Degree plus one year of pertinent experience. Supervisory experience preferred.

Compensation: $52,000

Responsibilities include:

· Supervise site-based and community-based activities in order to ensure that services are implemented and that they meet the needs, interests and capabilities of individuals and direct implementation of same.

· Provide supervision to staff, including direct participation and input into decisions regarding sourcing, hiring, orientation, training, scheduling, performance evaluation, discipline, and discharge of assigned staff.

· Actively participate in the support planning process for all assigned individuals to ensure objectives and activities are designed in accordance with each person’s interests. Attend all individualized team meetings.

· Train staff on individualized Staff Action Plans and teaching procedures. Ensure implementation of plans as designed.

· Provide direct oversight to the assignment, completion and quality of monthly progress notes for all assigned individuals. Ensure that monthly progress notes are completed timely in accordance with agency policies and regulatory standards.

· Evaluate delivery of program services and provide methods to upgrade program quality.

· Coordinate daily/weekly work and transportation schedules for people receiving supports.

· Perform Personal Outcome Measure (POM) Interviews as assigned to determine interests, skills and abilities of people supported. Assess and identify support, learning and training needs and generate related Action Plan.

· Drive agency vehicles for transportation of people receiving supports as required (pick-up, deliveries, community outings and hospital for treatment as needed).

· Monitor quality of services at community/job sites being delivered to individuals receiving support. Perform site visits and observations on a regular basis.

· Report incidents in a timely manner and complete all related notifications and documentation as required by agency policy and regulatory standards.

· Develop, implement, and conduct staff training programs and staff meetings. Conduct regular individual supervision with staff.

· Ensure the health and safety of every person receiving supports.

· Develop volunteer, internship and competitive employment opportunities.

· Act as liaison with other community organizations.

· Assist with personal care of persons receiving support as needed.

· Perform required bus duty functions as required.
