Community Specialist II *Up to $1000 Sign-on Bonus *

Full-time, Part-time, or Per Diem opportunities available

Up to $1000 in sign-on bonuses available- Earn $500 after 6 months of full-time employment and an additional $500 after 9 months of full-time employment.

Responsibilities include:

  • Teach people receiving supports functional skills as determined by each individual assessment.  This may include but is not limited to work readiness skills, behavior management, independent living and/or other skill development.
  • Monitor skills in a variety of settings, including but not limited to community-based work environments. 
  • Deliver hands-on instruction in community settings and collect data.
  • Provide transportation and travel training skills to the people receiving supports leading toward independence.
  • Determine aptitudes, interests and preferences and match people with community training experiences related to their desired outcomes.
  • Prepare and maintain all documentation pertaining to the people supported including daily data collection and monthly notes, as assigned. May include other required documentation for ACCES-VR, OPWDD, etc. depending upon assignment
  • Administer prescribed medications to people receiving supports after successful completion of AMAP medication administration course.
  • Drive Agency vehicles for transportation of people receiving supports as required
  • Act as liaison with other community organizations. 
  • Participate in planning meetings, as assigned.
  • Assess and identify learning and training needs of people.
  • Assist with personal care of people receiving support as needed.
  • Ensure the health and safety of all people receiving supports.

We Require:

  • High School diploma or Equivalency Exam; two years of college a plus
  • Minimum of one year of experience relevant to assigned location.
